Meredith Hardt, APN
Arthritis & Rheumatic Disease Management
“I believe that when a patient has all of the information, they can make the best decision for their health. My goal is to provide that information and then work with the patient to develop the best possible treatment plan for their disease and life situation. I love improving pain and helping patients regain independence.”
Meredith received her bachelor’s degree in nursing from Saint Mary’s College in Notre Dame, Indiana followed by a master’s degree in nursing as a family nurse practitioner from Vanderbilt University in Nashville, Tennessee.
Meredith began her career working in wellness and primary care for three years. During that time, she developed a fondness for treating patients with chronic disease and musculoskeletal injuries. This led her to rheumatology. She worked at the University of Chicago in the adult rheumatology clinic for over 4 years until coming to Greater Chicago Rheumatology. Her areas of greatest interest include Osteoarthritis, Gout and Rheumatoid Arthritis.
Meredith has been a preceptor for nurse practitioner students from the University of Illinois Chicago and Resurrection University.
